Health Department Job Openings 

Home Care RN – Full-time – 40 hours/week - The Home Care RN will be cross trained to participate in the multi-disciplinary care of Home Health and Hospice patients following the plan of treatment prescribed by the patient’s physician. Regular schedule is Mon-Fri, 8 a.m. – 5 p.m. with participation in call rotation. RN experience preferred. IL RN License required. IL driver’s license required. Position reports to the Home Care Coordinator.


Breastfeeding Peer Counselor – Part-time – 10 hours/week. A Breastfeeding Peer Counselor is a paraprofessional support person who gives Basic breastfeeding information and encouragement to WIC pregnant and Breastfeeding mothers.Position reports to the HFI/Peer Counselor Coordinator. Per grant guidelines the Breastfeeding Peer Counselor qualifications are as follows:


  • a woman of the community, with similar characteristics (i.e. age, race, cultural ethnicity, education and socioeconomic level) of WIC participants. (Professional staff, older women or women of disparate backgrounds or characteristics are not appropriate as peer counselors.)
  • previous or current WIC participation, or other women from the community with breastfeeding experience and an enthusiasm for breastfeeding may be recruited, hired, and trained to be a peer counselor
  • must have breastfed at least one baby (within the last five years), but does not have to be currently breastfeeding.

Early Childhood Development Specialist - Full-time @ 32 hours weekly - A certified parent educator implements the Parents as Teachers (PAT) model, emphasizing parent-child interaction, development-centered parenting and family well-being in their work with families. HS diploma with minimum of 2 years supervised experience working with young children and/or parents minimum requirement. Preferred qualifications include Bachelor’s degree in early childhood education, social work, health, psychology, or a related field with 60 hours in early childhood development. PAT certification required and may be acquired after hire. Position reports to ISBE Grant Coordinator.

Social Worker - PRN to assist with growth - The Social Worker is responsible for psycho/social assessments of CCHD Home Care clients/patients.  LCSW required.  Position reports to the Hospice Coordinator.
