Community Health
The Clay County Health Department facilitates a community health planning process known as the Illinois Project for Local Assessment of Needs (IPLAN). We use the “Mobilizing for Action through Planning and Partnership (MAPP) framework and emphasize a community-driven, community-owned approach.
Health for Generations (H4G) Coalition: The Health for Generations Coalition is a group of community leaders who explore data to help understand community needs, gaps, and identify priorities. H4G seeks broad community participation, and all meetings are open to the public. The group meets quarterly. Interested in joining the coalition? Our vision for Clay County:
The Clay County Health for Generations Coalition envisions an environment that supports a healthy lifestyle and health prevention activities that start early. We envision a public health system that is diverse and collaborative that encourages broad participation and effectively reaches the whole population, including the most vulnerable members of our community.
